The above card (click on image for .pdf file to print) is designed to be printed on a business card sheet of ten, to be kept with your license at all times to facilitate emergency personnel and your loved ones in the event you are incapacitated or killed in an accident. Be sure to include phone numbers for both primary and secondary emergency contacts that know how to contact other members of your family. Include any life threatening allergies you have for first responders to be aware of. State the location of your health care proxy, living will, power of attorney so your family may locate them quickly if needed. Also provide the name and address of the attorney that has your last will & testament on file. Powers of Attorney expire upon death. After the card is filled out have it laminated, or cover it with packing tape front and back so that it remains legible if it gets wet. Be Safe! Be Prepared! Ride Free!
